Using the instructions below, complete:
1. Case 3.5 Vicki Sims (5 points)
2. Case 6.1 Gloria Kuhlenschimidt (5 points)
For each case, please use the following headings and information to guide your assignment.
Part I. Presenting Problem and Client Background (worth 1 point)
Provide a wellwritten
description of the clientâs demographic background, presenting problem (why they are seeking help),
clientâs psychosocial issues and functioning, clinical history and any cultural issues that may bear on the diagnosis. Take
special care to ensure that the information included is relevant to the clientâs functioning and/or diagnostic formulation.
Part II. Diagnostic Formulation/Summary (worth 1.5 points)
Write a short paragraph that provides your rationale for your selected diagnosis or diagnoses. Take special care to ensure
that you understand the symptoms that are presented and that they manifest correctly in your diagnosis in part III. Be sure to
mention any aspects of the clientâs behavior or presentation that may bear on the diagnosis (mental status exam).
Part III. Diagnosis (worth 1.5 points)
Provide a complete diagnosis. Please include the appropriate DSM5
disorder coding number along with the diagnostic

specifiers when appropriate. The diagnosis should be fully supported by your diagnostic formulation.
Part IV. Intervention (worth 1 point)
Provide a paragraph that delineates the possible treatment and referrals you may utilize in this situation with this client, and
